The FIND Community Team is a South London Partnership (SLP) service hosted and delivered by Oxleas NHSFT. The FIND Community Team is a multi-disciplinary team consisting of nursing, occupational therapy, speech and language therapy, social work, four psychological therapists and psychiatry. The team work alongside local community LD, mental health, and forensic mental health teams across the region, providing advice and consultation, or working jointly, with local teams to ensure service-users clinical and psychosocial needs are met. This can include direct work with service-users and/or their family or carers. The service can consider referrals for service-users already living in the community, or for those in inpatient settings that are within six months of discharge to the community. The FIND Community Team are passionate about supporting service-users to live in the least restrictive setting necessary, and close to their local community.
